Joimax ® is a leading developer and supplier of integrated systems for minimally invasive endoscopic spine surgery. Reliable endoscopic systems allow you to cover a wide range of indications when using TESSYS ® (transforaminal access), iLESSYS ® (interlaminar), CESSYS ® (access to the cervical spine) for decompression procedures, MultiZYTE ® RT (for example, for rhizotomy) or EndoLIF ® and Percusys ® for minimally invasive endoscopic stabilizations.
Transforaminal Endoscopic Surgical System
Suffering from herniated discs is so prevalent that it is qualified as a “common disease”. Males are affected twice as often as females. The TESSYS®method uses the lateral transforaminal endoscopic approach, which is, in general, a “gentle surgery”.
The TESSYS® surgery
Using the TESSYS® approach the surgeon resects the herniated disc through the foramen intervertebrale. Thus, a natural foramen to the spinal canal is used. With the TESSYS® technique this is widened step by step using specialized reamers and instruments. Under endoscopic view sequestered disc material is removed completely and directly through the foramen.
For the patient – as gentle as possible
A TESSYS® operation can be performed in prone or lateral position. Patients can receive either general anesthesia or MAC. If the surgery is performed under MAC, patients are able to react immediately upon possible contact with neural elements. Many patients get up just a few hours after the surgery.iLESSYS® – Interlaminar Endoscopic Surgical System
The iLESSYS® surgery: minimally invasive, maximally gentle
The iLESSYS® method uses the endoscopic interlaminar access for the removal of herniated disc material or the treatment of spinal stenosis. This method is less traumatic than the conventional microsurgical technique.
The small diameter of the joimax® LaminoscopeTM and working tubes preserves the tissues. Due to the extraordinary optics a brilliant view on all structures is provided. The iLESSYS® method is a real minimally invasive operation technique for pathologies in the dorsal area of the spinal canal.
Patient benefits
The surgery can be done using either general anesthesia or MAC. If MAC is used the patient remains responsive during the surgery, thus you can react directly in the case of contact with the nerves. Furthermore, the risks and side effects of general anesthesia are avoided.
An interlaminar endoscopic approach for the treatment of stentoses and herniated discs: The iLESSYS® surgical technique
A careful planning of the main point of access to the herniated disc is crucial for positive surgical results. Magnetic Resonance Imaging (MRI) and/or computer tomography (CT), as well as multiple conventional x-ray images have to support the iLESSYS® surgery.
In general, the patient is positioned in prone position. Like with the transforaminal system TESSYS® also the interlaminar access is reached by tissue sparing graduated dilatation and subsequent placement of the working tube. Under endoscopic view the ligamentum flavum will be opened to enter the spinal canal. Depending on the indication no or just a little of bony material will be removed.The CESSYS® Concept
The CESSYS® methods are exceptionally minimally invasive endoscopic surgical techniques for decompression / deherniation within the cervical spinal canal and as such far less traumatic than microsurgical techniques. The optics of joimax® endoscopes provide an excellent direct view of all structures directly at the site of the surgery.
Besides CESSYS® Dorsal using the posterior access, there is another member of the CESSYS® family, the CESSYS® Ventral system using the anterior, intradiscal access to the spinal canal. Together both systems realize the joimax® 360° decompression philosophy for the cervical spinal canal. While CESSYS® Ventral with its smaller endoscope and instruments is better suited for removal of small and soft disc herniations in the ventral spinal canal, CESSYS® Dorsal is perfectly suited for treatment of dorsal pathologies such as osseous decompression cases.
For bony decompression within the cervical spinal canal
The working channel diameter of CESSYS® Dorsal endoscope is 4.7 mm. This allows the use of large punches, forceps and Shrill® shaver blades with outer diameter 4.5 mm for effective removal of hard tissue like bone or hypertrophic, sclerotized ligamentum flavum. At the same time, the endoscope is still small enough to be ideally suited for working within the cervical spinal canal.
This makes the system particularly suitable for bony decompression (stenosis). The working length of 68 mm and the 15° direction of view of the endoscope makes it excellent for application in the cervical spine using the dorsal approach. Due to irrigation in endoscopic procedures, the visualization is constantly optimized and not compromised by bleeding.
The endoscopic system for the treatment of herniated discs in the cervical spine
CESSYS® allows the minimally invasive treatment of cervical disc herniations. It uses an anterolateral approach through the disc space directly to the site of herniation. The intervention can be carried out either under general anesthesia or under monitored anesthesia care (MAC). MAC is generally associated with less anesthesia risks and offers the advantage of immediate patient feedback if there is any contact with the neural structures.

Dorsal and dorso lateral endoscopic treatment of spinal canal stenosis
As a further development of the joimax® iLESSYS® technology, iLESSYS® Delta is suitable for the dorsal and dorso lateral treatment of the central spinal canal stenosis. With the iLESSYS® Delta system, under endoscopic view a large area can be decompressed using the interlaminar access. For this purpose, various instruments are available, which can be inserted through the new Full HD LaminoscopeTM with a working channel of 6 mm.
The surgical method is particularly gentle due to the gradual dilation in the access of the pathology tissue. Under endoscopic view, only compressive structures are removed, e.g. hypertrophic ligamentum flavum or lamina.
The new Member of the iLESSYS® Family
The iLESSYS® method is an endoscopic minimally invasive surgical procedure, which uses the interlaminar access path to remove disc herniation or treat stenosis. With the new iLESSYS® Pro instrument set, joimax® expands its iLESSYS® system family for the interlaminar endoscopic access. The endoscope and all instruments for access and resection have been optimized for decompression within the spinal channel in collaboration with Dr. Christoph Hofstetter. The new set exhibits excellent properties especially for ipsi- and contralateral intraspinal bony decompression and deherniation. Another focus of development was the aspect of surgical efficiency, streamlining of the procedure and improvement of ergonomics.
Perfect for intraspinal, ipsi- and contralateral decompression and deherniation
The working channel of iLESSYS® Pro LaminoscopeTM is 4.7 mm. This allows the use of large punches, forceps and shaver blades for effective removal of hard tissue like bone or hypertrophic, sclerotized ligamentum flavum. At the same time, the endoscope is still „small enough“ to be perfectly suited for working within the spinal canal. The working length of 150 mm and the 15° direction of view makes it excellent for contralateral procedures without any compromises for optimal ipsilateral application. Due to irrigation in endoscopic procedures the visualization is constantly optimized and not comprimized by bleeding.
